A new study suggests that older adults who lived in socialist East Germany before the 1989 reunification show better cognitive health in later life and slower mental decline than their West German peers. The researchers point to the former East German educational system, with its longer compulsory schooling and universal early childcare, as a possible reason.
Researchers Ariane Bertogg of the University of Konstanz and gerontologist Jeffrey E. Stokes of the University of Massachusetts Boston conducted the analysis, framing the long-standing division of Germany as a natural experiment to examine how different political systems shape cognitive development across a lifetime.
After World War II, Germany was split into two nations with divergent political and economic models: the German Democratic Republic in the East, a socialist state with a planned economy, and the Federal Republic of Germany in the West, a capitalist liberal democracy. The two halves reunited in 1989, merging into a single legal and economic system.
The East-West contrast has often been linked to physical health and life expectancy, where East Germans historically lagged behind. The new study asks whether those differences extend to the aging brain and cognitive health in later life.
Two competing ideas guided the researchers: one suggested that West Germans might outperform cognitively due to stronger economic growth, better nutrition, and more civil liberties; the other proposed that East Germans could have an advantage from their state-funded education system.
East Germany mandated ten years of compulsory schooling, compared with eight or nine years in the West, and universal early childcare along with a less socially divided schooling structure. Educational exposure early in life is a primary driver of cognitive reserve—the brain’s ability to build dense neural connections to compensate for aging-related damage.
The team analysed data from the Survey of Health, Ageing and Retirement in Europe, focusing on 5,231 older adults who lived in either East or West Germany in 1989. Participants were aged at least 50 at the time and were interviewed multiple times over an average of four years.
The survey included four standard tests used to evaluate brain health and predict dementia: working memory (recalling a list of ten words), executive function (naming as many animals as possible in a set time), orientation (knowledge of the current date and day of the week), and numeracy (repeatedly subtracting seven from one hundred).
Researchers employed statistical models to map each person’s cognitive trajectory, adjusting for childhood health, household income, and overall educational attainment. They also drew on a retrospective module to capture childhood health, perceived maths and language skills relative to peers, and the number of books in the home.
The data showed that individuals who lived in East Germany prior to reunification performed better on all four cognitive tests and experienced a slower rate of decline as they aged. West Germans, by contrast, exhibited a steady decline in immediate memory and verbal fluency, while East Germans tended to maintain stable scores or improve in these areas.
East Germans also showed less steep declines in orientation and numeracy, with the advantage particularly pronounced in men. The findings present a notable paradox in aging research, given higher cardiovascular disease rates and earlier mortality historically observed among East Germans.
The researchers note that the early benefits of an egalitarian, intensive education system may offset some physical disadvantages linked to a lower standard of living, at least in terms of cognitive aging. They caution that the dataset could not measure direct social or environmental stressors that might have contributed to outcomes.
Looking at reunification by life stage, the study found the memory and verbal fluency gap was widest among those in their forties at the time of the Wall’s fall. For numeracy and orientation, the advantage for East Germans was strongest for those who experienced reunification in their twenties. Among those nearing retirement in 1989, the cognitive differences were largely absent.
The researchers interpret these patterns as evidence of critical periods in adult development, where entering the workforce during reunification may have helped younger East Germans maximise the benefits of their schooling within a changing economy, while mid-life upheavals in West Germany may have amplified mental health challenges.
Beyond education and economics, other differences between the two states—such as environmental toxin exposure and social climate—likely contributed to the observed outcomes. The West’s higher car ownership correlated with greater exposure to lead from gasoline, while East Germany’s use of informants and secret police is believed to have shaped social trust, though the study could not quantify these effects directly.
The authors emphasise that further research is needed to explore similar patterns in other post-socialist and capitalist European contexts and to track clinical diagnoses of Alzheimer’s disease and other dementias, rather than relying solely on cognitive test scores. They caution that high-performing individuals can reach the ceiling on some tests, potentially masking early decline in real-world settings.
